NeuroPointDX.com Terms of Use Agreement

This Terms of Use Agreement constitutes a legally binding agreement made between you, whether personally or on behalf of an entity, and NeuroPointDX and its affiliated companies concerning your access to and use of the neuropointdx.com website. The website provides the following services: Information related to the NeuroPointDX ASD (NPDX ASD) Test Panel and company services and provided forms for use with the NeuroPointDX ASD Test Panel. Supplemental terms and conditions or documents that may be posted on the website from time to time are hereby expressly incorporated into this agreement by reference.

You accept and agree to be bound by this agreement by acknowledging such acceptance during use of the website. If you do not agree to abide by this agreement, or to modifications that NeuroPointDX may make to this agreement in the future, do not use or access the company services or the website.

Patient Representations

Regarding NPDX ASD Test Patient Requisition Form Submission

By using the company services, you and the ordering physician represent and warrant that:

1. All information submitted is truthful and accurate;
2. You will, together, maintain the accuracy of such information; and
3. Your use of the company services does not violate any applicable law or regulation.

You also agree to: (a) provide true, accurate, current, and complete information about yourself, your child, or your patient, as guided by the Test Requisition Form, and (b) maintain and promptly provide data updates to keep it true, accurate, current, and complete.

If you provide any information that is untrue, inaccurate, not current, or incomplete, or NeuroPointDX has reasonable grounds to suspect that such information is untrue, inaccurate, not current, or incomplete, then NeuroPointDX has the right to discard your or your child’s Patient Information and refuse delivery of services provided to the ordering physician.

Privacy Policy

We care about the privacy of our customers and their physicians. Please review the NeuroPointDX Privacy Policy. By submitting a Test Requisition Form through the ordering physician, you are consenting to have your or your child’s patient personal health data obtained and processed in the United States. By using NeuroPointDX services, you are consenting to the terms of our Privacy Policy.
